SCAM REPORT FORMAT - REQUIRED TO MAKE A VALID REPORT

Hello, please follow this format if you have been scammed, or if you feel somebody is a scammer based on an interaction you had with that person. Please, fill the ENTIRE format regardless of what you believe will happen as outcome of the report. The information is MANDATORY.
